Apple Valley Christian Care Center's skilled nursing facility offers both short term and long term care. As a 99-bed fully licensed skilled nursing facility, Apple Valley Christian Care Center features comprehensive medical care in semi-private and private rooms and a Rehabilitation Unit. Hospice care is available for terminally ill patients and their families.
Residents receive care 24-hours-a-day by licensed nurses and certified nurse assistants. Our staff utilizes state-of-the-art equipment combined with methods that are managed in a caring, healing environment.

REHABILITATION SERVICES
Our team of devoted, licensed therapists provide physical, respiratory, occupational and speech therapy for patients who are recovering from:

- Strokes
- Heart Attacks
- Neurological Disorders
- Illnesses, Injuries and Disabilities
- Physical, Occupational, Speech Therapy
Our rehabilitation department's therapy staff has specialized equipment used for strengthening exercises, educating and retraining patients to stand up, walk and conduct daily life activities.

SOCIAL SERVICES AND DISCHARGE PLANNING SERVICES
Our Social Services staff assists residents with transportation to and from appointments, setting up appropriate home health care and providing resources for obtaining medical equipment such as walkers, wheelchairs and shower chairs.
Prior to discharge our social services department will work with the resident, family and resident’s physician to provide the necessary assistance for the transition back home.
